Bright Futures
Bringing healthcare needs into our neighborhoods

One of the most direct and effective ways we extend critical healthcare beyond our walls and boundaries is though our Bright Futures Mobile Van, which helps low- and moderate-income families gain access to healthcare.

You/your child may use Bright Futures Mobile Van services if you need healthcare and:

  • Do not have health insurance.
  • Are a low- to moderate-income family.

Bright Futures - Your Family Needs Healthcare Regularly

Your child gets the following services during his/her exam:

  • Health check-ups, including a physical exam, vision and hearing, blood and urine test, blood-lead testing, TB skin test, immunizations (when necessary) and dental screening.
  • Medical, nutritional and developmental history.
  • Other tests, as necessary.
  • Health education.
  • Referrals for additional medical care if needed.

Your baby should see a pediatrician or the nurse practitioner on our van at these ages:

2 weeks, 2 months, 4 months, 9 months,
12 months, 15 months, 18 months,
2 years, 5 years, 7 years

Adult Services available:
